Why do goverments seem so dysfunctional? As a goverment employee myself I have seen the mismanagement.?

It seems in my agency when there are productive issues to be solved, they rather single out one employee and focus all their attention on that one person for discpline, rather than view the problem as much larger than the fault of any one person, but an organizational issue. As a result the real issue never gets resolved.
